Accounting for Patient Engagement in Randomized Controlled Trials Evaluating Digital Cognitive Behavioral Therapies

Background: Cognitive behavioral therapy (CBT) can be a useful treatment option for various mental health disorders. Modern advances in information technology and mobile communication enable delivery of state-of-the-art CBT programs via smartphones, either as stand-alone or an adjunct augmenting traditional sessions with therapist. Experimental CBTs require careful assessment randomized clinical trials (RCTs). Methods: We investigate some statistical issues RCT comparing efficacy experimental intervention disorder against the control. Assuming linear model outcome patient engagement influential covariate, we two common approaches to inference—analysis covariance (ANCOVA) two-sample t-test. also study sample size requirements described setting. Results: Both ANCOVA t-test are appropriate inference on difference at average observed level engagement. However, produces estimates lower variance may more powerful. Furthermore, unlike t-test, allows one perform comparison levels other than study. Larger sizes required ensure experiments sufficiently powered if is interested effects different Conclusions: proper adjustment should used Uncertainty patterns taken into account design stage.
